Can a pre-accident be ‘investigated’? No, it can’t. Don’t fall for the con!
Are accidents normal? No, they are not, despite the slogans of HOP. Does blame fix things? Yes, it does. Don’t buy the con.
Can you ‘investigate’ fallibility? Yes, you can, but only in the sense that you can seek to know about human mortality, imperfection and vulnerability.
However, you can’t investigate something that doesn’t happen. Don’t fall for the con!
The language of ‘pre-accident investigation’ is nonsense (https://www.humandymensions.com/product/seek-investigations-a-semiotic-method/). Such language and discourse has more in common with a psychics convention that understanding the nature of risk.
Humans can only ever see the cause or an accident after the accident happens. You can only ever understand an event after the event has happened.
There is no such thing as human infallibility or omniscience. This is the sense of fallible living. As Kierkegaard rightly declared:

Unfortunately, at the foundation of Safety is a deep-seated lack of critical thinking and a delusion that safety is about compliance, in everything, especially ideas. The opposite is the case. Critical thinking starts with the contestation of ideas NOT compliance to ideas nor the seduction of slogans. Don’t fall for the con.
Spin, propaganda, indoctrination and slogans are the foundation of a con.
